Billing Dispute Between Workers Comp Doctor and Insurance Company

There is a billing dispute between the Dr and WC. WC has offered me a settlement with out the bill resolved. It is at 10% PPD. The Dr wont release me rating till the billing is resolved. Should i wait however long for the bill to be resolved or should i go forward with the settlement. And will I be liable for the bill after settlement? I believe i have been treated fair the whole time, but im worried that my actual rating might be higher than 10%

why don't you want to ask for another doctor's rating?
can't the report be subpoena'd?
