WCMSA Annual Self-Attestation Form

My workers compensation claim was made in the State of: ILLINOIS

I mailed my annual accounting form in on March 16th and a week later it was sent back to me saying forwarding address had expired. I used the same address I have been using for the past 4 years to Detroit, Michigan but sometime last year they changed their address to Oklahoma City, OK. So if anyone on this board has to send in an annual attestation form the correct address is: MSRPC P.O. Box 138832 Oklahoma City, OK 73113-8832.

Thanks for the information. It is not very common for individuals who actually have a WCMSA to post very often.
