Those numbers mean just what it says... "Body part"... ie, arm/upper extremity, leg/lower extremity, neck, back, etc. Under the rating system, the AME 5th ediciton for rating permanent disability, the Dr evaluates your condition, based on the body part claimed injured, and converts that number to a final PD/WPI, Whole Person Impairment rating.

Other than that... you haven't provided enough info to tell what your situation may be. Or as SH said, what form you are looking at.

Just go to page 8, like the instructions say, and you'll find the body part the number correlates to.
    body part 1: 419 body part 2: 512 body part 3: 449 body part 4: 519 other body parts 400 838 882 818 got these numbers on a dwc-ca form
So far...the answer remains the same...those numbers identiy the body parts you claimed injury to.

The form is just what it says... "Document Cover Sheet".
