    Default Denied Work Injury

    I received a letter yesterday from Gallagher Bassett they have denied my claim. Futher the letter stated when they receive a claim from the Industrial Commission they will tell them the claim has been denied. I thought the Industrial Commission had already received the claim, but I WAS WRONG AND MISS LEAD FROM CA.

    How do I get the paperwork for filing claim to the Industial Commission? I called the Arizona number and no none had returned my call.

    I have contacted an attorney with a 10 rating on AVVO, so I am hopeful he will be able to help me. Injury on 12/21/2012 and just now receiving letter from CA!!!! Wow really mislead me all along!!! The doctor's WC person handling all WC claims told me all paperwork had been sent in.

    Who is responsble to make sure the Industrial Commission is notified? I am in such an idiot!!!!!

    You ultimately are responsible. That is why you need to hire a lawyer.
