    Default Medical Waiver and Consent Form

    Received a form to sign and return to comp insurance. Do I absolutely have to do this? I'm not hot on signing things that have "Fill in the blanks later" on them. Supposedly it's to get past medical history.

    Yes, you have to sign that release or they can deny the claim.
    To be sure, check with your lawyer before doing anything.
