So who would be my employing agency? Do i just forward the CA-7 to OWCP or who? Thank you in advance.
The agency you worked for at the time of your injury.
Submit the CA-7 to your supervisor, from what I see if you're still not employed you can file it with the Office of Workers' Compensation Programs (OWCP)
You better call the Office of Workers' Compensation Programs (OWCP) and ask for help if you don't know what you're doing, if you make a mistake in filing it may delay your claim for a long time.
Read the instructions at the bottom of the form.
INSTRUCTIONS FOR COMPLETING FORM CA-7
EMPLOYEE (or person acting on the employee's behalf) - Complete sections 1 through 7 as directed and submit the form to the employee's supervisor.
SUPERVISOR (or appropriate official in the employing agency) - Complete sections 8 through 15 as directed and promptly forward the form to the
OWCP.
